Predicting peanut (and food) tolerance

There is a lot of talk nowadays about food allergies and never have we had so many modalities with which to deal with them, from food oral immunotherapy to Palforzia to omalizumab (Xolair), the biologic.

Recently peanut allergy tolerance was studied (Parker KM, et al. Longitudinal peanut and Ara h 2 specific-IgE, -IgG₄, and -IgG₄/-IgE ratios are associated with the natural resolution of peanut allergy in childhood. Allergy. 2024 Jul;79(7):1868-1880) in part because there is less than 20% likelihood of outgrowing peanut allergy with age.  The question of the research was how do we know the person has outgrown their allergy, preferably using lab studies, as food challenges can be scary.

Now we have some better biomarkers to assess persistence v. tolerance of peanut with age.  It was found that children with persistent peanut allergy had increasing peanut IgE (the allergic antibody or as I call it the “allergic flag) and their peanut IgG4 (a flag associated with tolerance) spiked but then plateaued with age.  On the other hand, children whose peanut allergy resolved, had a decrease in peanut IgE whereas the peanut IgG4 levels continued to increase until at least 10 years old.
